Live Well provides each client with a copy of their confidential disclosure agreement, and outlines a service agreement between client and psychotherapist. Live Well abides by Federal and State laws regarding client confidentiality and privacy.
All evaluation and treatment information is confidential, and can only be released to individuals, or agencies with your written consent. Some exceptions apply, such as mandatory reporting in the case of child or elder abuse, or when a client presents as a danger to self, or others. A Court may also order release of information during the course of a legal proceeding.
